The four-column framing is right and it is under-used. Most policy arguments about this collapse the moment you ask which adversary the person is worried about, and the answer is almost never the same one across the room. Where it gets difficult is that the platform column is the one users have least ability to verify. You can reason about the state and your employer. You cannot audit whether the company kept what it said it discarded, and every promise in that column is ultimately a promise about behaviour rather than a property of the system.
Someone succeeds, works backwards from the outcome, and sells the routine. The people with identical routines and no book are not in the sample.
Radio still does it properly because it has to. Television gained pictures and lost the language.
It depends what you count. Operational safety yes, but the metric quietly excludes the plants cancelled halfway through, and those overruns are what actually stops it being built.
