@tomasn depends which failure you mean. For maintenance the timescale story is clearly right. For the timetable collapses it is not — those were failures of a specific process on a specific date, and the people involved knew weeks in advance. Worth keeping both. One explains the slow rot, the other explains the sudden breaks, and conflating them is how we end up with reviews that recommend reorganising the org chart.
